Almost Home provides Rapid Re-Housing (RRH) to households experiencing literal homelessness in Adams and Southern Weld Counties. Once enrolled in the program, households receive ongoing supportive, home based case management services to help ensure ongoing housing stability and short-term rental assistance. Referrals for this program come directly from local CoC Coordinated Entry systems.

 

All Almost Home Rapid Re-Housing Programs are closed referral programs and we cannot accept partner or self-referrals.
